Policies

 

1. OakGate Medical is committed to providing a safe and respectful environment for our staff and patients. There is a zero-tolerance policy for threatening or inappropriate behaviors towards doctors, staff, or any other patients. This includes, but is not limited to, swearing, yelling, threats or threatening behavior, rude or demeaning comments, sexual harassment, and discriminatory remarks. Any patients engaging in such behaviors in person, by telephone or video, will be immediately discharged from the practice.

 

2. Appointments are booked to address one or two medical issues per visit in order to thoroughly address each concern and allow time for Q&A. If you have more than two medical issues to address, please let the doctor know at the start of the appointment so that you can decide together which issues will be addressed now and which will be saved for a future visit.

* All family members that need to be assessed by the physician require their own appointment.

 

3. The Doctors at OakGate Medical provide enough medication that is safe and indicated for the medical issue it is treating. Patients are responsible for checking when they will run out of medications and booking an appointment at our office before they run out. If you are out of refills at the pharmacy that means it is time for an appointment to ensure the medication is being tolerated and that it is still effective. Small supplies (4-7days) can be faxed out to hold you until you are able to book an in office or virtual appointment (depending on prescription type).

 

4. A valid OHIP card must be presented at each visit to receive medical care, including virtually. If you do not have a valid Health card, you will be charged for the appointment according to OMA rates.

 

5. Completing referrals to specialists takes a significant amount of time. Multiple referrals to specialists for the same issue will not be completed, as this burdens the healthcare system and increases wait times.

 

6. Our email address is not monitored regularly. If you have an urgent medical concern or are seeking medical advice, please call the office during regular business hours to request an appointment. If this is a medical emergency, call 9-1-1 or head to your local emergency department. 

 

7. As COVID-19 is still very much a health concern, masks are mandatory within our office space. Mask wearing continues to be an effective public health measure for reducing the spread of COVID-19.

 *Some exceptions apply, speak with your doctor at the time of your appointment.

 

8. OakGate Medical staff and doctors reserve the right to change your in-office appointment booking to a virtual booking if you have been exposed to COVID-19 or have tested positive for COVID-19.

 

9. OakGate Medical does not service patients who are not registered to either Dr. Adriana Dragan or Dr. Dusanka Gvozdic. This is an appointment only clinic and walk-in patients will not be serviced. You can request an appointment online at www.oakgatemedical.ca, on your Medeo App, or by calling the office during normal business hours.

 

10. Not all services offered at OakGate Medical are OHIP insured. There is a price list posted on our website for all non-OHIP insured services such as sick notes, short term disability forms, and chart transfer fees. A member of the administrative team can also supply you with this information.

 

11. Patient Forms: Patient care is number one at OakGate Medical therefore any forms (insurance, work, tax, etc.) submitted may take at least 7-14 days to complete. To help this process along please ensure that you have signed the form and that all your personal information (name, address, etc.) has been filled out. *Please indicate whether the form can be sent directly to the requesting party or if you would like it returned to you directly. According to OMA fee schedule, there will be a charge for the completion of most forms.
